What data we process

Through the contact form we process only what you enter yourself:

• your name;
• your email address;
• your telephone number, if you enter one — this field is optional;
• your preference for how we get back to you;
• the subject you select and the text of your message;
• the language you are using the site in, and the page you sent the form from.

We also process your IP address temporarily, to stop the form being abused for spam. We set no tracking or advertising cookies.

What we use your data for

We use this data solely to answer your question, to arrange an appointment with you, and to prevent abuse of the form. We do not use your data for advertising, we do not sell it, and we do not share it with third parties for their own purposes.

On what legal basis

• Consent — you tick the box yourself to allow us to contact you. You can withdraw that consent at any time.
• Performance of, or steps towards, a contract — where your message concerns arranging an appointment.
• Legitimate interest — securing the form against automated abuse.

How long we keep your data

We keep contact form data no longer than is necessary for the purpose you sent it for: answering your question or arranging your appointment. It is deleted afterwards. If you become a patient, your treatment records are subject to the statutory retention periods of the Dutch WGBO; those records are created at the practice, not through this website.

Who processes your data on our behalf

Two service providers are involved in running this website:

• Vercel Inc. — hosting the website and handling the form request.
• Resend (Plus Five Five, Inc.) — delivering your message to the practice's mailbox and sending your confirmation email.

These parties process your data solely on our instructions and not for their own purposes. Both are established in the United States, which means your data may be processed outside the European Economic Area. Such transfers are subject to the safeguards in Chapter V of the GDPR, such as the European Commission's Standard Contractual Clauses.

Your message ultimately arrives in the practice's own email inbox, which is hosted by an external email provider.

Security

• All traffic to and from this website is encrypted over HTTPS.
• The form is protected against automated abuse and rate-limited per IP address.
• The content of your message is not written to server logs.
• No form data is passed to analytics or advertising services.

Do not send medical or urgent information through this form

The contact form is not intended for medical data. Do not share symptoms, diagnoses, medication or other health information through it. The form is not monitored around the clock. If you have acute pain, an accident or another urgent dental situation, call the practice, or outside opening hours the emergency dental service for your area.

Your rights

Under the GDPR you have the right to:

• access your data;
• have inaccurate data corrected;
• have your data erased;
• have processing restricted;
• object to processing;
• receive your data in a commonly used format;
• withdraw consent you have given.

Send your request to dentacareosdorp@hotmail.com. We respond within the statutory period of one month. To avoid disclosing data to the wrong person, we may ask you to identify yourself.

Complaint to the Dutch Data Protection Authority

If you disagree with how we handle your data, we would rather hear it from you first. You also always have the right to lodge a complaint with the Dutch supervisory authority, the Autoriteit Persoonsgegevens, at autoriteitpersoonsgegevens.nl.
